Moving Forward

We know that when you care about a young person in need, you want him or her to be able to move into Mercy Home as fast as possible.  Our admissions process is designed to ensure that each youth who comes to live at our Home is best served by the resources we provide.

Here is a list of documents that you can provide that can help us keep the admissions process on track and moving forward:

  • Complete hospital records if the youth has ever been hospitalized for behavioral or psychiatric reasons
  • Legal records (if applicable)
  • School records including:
    • Standardized test scores (Explore, Plan, ISAT, Terra Nova)
    • Unofficial school transcripts (including all earned credits & year in school)
    • Current class schedule with grades in progress
    • Copy of current Individualized Education Plan (IEP) (if applicable)
    • Attendance record (noting all absences and tardies)
    • Medical records (physicals, dental records, immunizations)
    • Behavior records (detentions, suspensions, conduct reports, etc.)
  • Birth certificates and Social Security cards. We do not need birth certificates until the day the youth moves in. However, if you think you will need to obtain a new birth certificate, please do so as soon as possible. We cannot schedule a move-in date without a birth certificate.
  • Proof of guardianship for youth under the age of 18, such as:
    • Court documents
    • Tax forms
  • Medical insurance—Anyone under the age of 18 who does not have medical insurance should sign up for All Kids, the Illinois healthcare program for children. If you do not have insurance, we will work with you to schedule a physical at a facility where you can sign up for this program.
